◆Depending on the situation, we may contact parents/guardians separately.
◆ For regular absences and bereavement leave, please apply using the "Official Absence, Bereavement, and Injury Contact Form." (There is no need to apply using this form.)
◆If you test positive for influenza or COVID-19, please submit documentation that proves you were infected after returning to school (medical statement or hospital/prescription receipts are acceptable) to the Academic Affairs Office.

Contact form for absence from work, bereavement, and injury

◆Please apply for official leave as early as possible, up until the day before, and for bereavement leave, please apply as early as possible on the day of the event.
◆If you are absent due to an official reason, be sure to inform your homeroom teacher in advance of the "date (time limit)", "reason", and "related documents if necessary" you wish to apply for, and enter them only after receiving their approval.
*Even if you think your absence is authorized, it may not be so, so be sure to get permission from your homeroom teacher.
◆Please refer to the appendix to the "Internal Regulations Concerning Academic Affairs" to see whether an absence constitutes an official absence.
◆The maximum period of bereavement, excluding the number of days for travel, is 7 days for parents, 3 days for grandparents and siblings, and 1 day for great-grandparents and aunts and uncles.
*When adding the number of days or hours required for a round trip, the maximum is 4 days.
◆If you test positive for influenza or COVID-19, please apply using the "Absence/Lateness/Early Leaving Notification Form."
